Crestview Dental (“we”) is a dental clinic at 215-1568 Merivale Road, Ottawa, ON K2G 5Y7. This page explains how we handle personal information on this website and when you book a visit. Health records created at the clinic are also protected under Ontario’s Personal Health Information Protection Act (PHIPA).
When you book online, we (and our scheduling provider, Cal.com) collect your name, email, phone number, the visit type and time you chose, and any notes you add. If you use the contact form, we collect your name, phone, email, and message and email that to the clinic. If you call, email, or visit in person, we collect the details needed to identify you, contact you, and provide care — for example insurance information, medical history, and treatment records.
The website itself does not run advertising trackers. Your browser may still send standard technical data (such as IP address and pages viewed) to whatever host serves this site.
We use this information to schedule and confirm appointments, send reminders, provide dental care, bill you or your insurer, and answer questions. We do not sell your information.
Online bookings are processed by Cal.com so the time can be held on our calendar. They handle that data under their own terms and privacy policy. We may also share information with your insurer when you ask us to submit a claim, with specialists involved in your care, or when the law requires it.
Dental records are kept for the period required by Ontario dental regulations. Booking details stay as long as we need them to manage your visits.
You can ask to see, correct, or update personal information we hold, or ask how to cancel a booking. Use the link in your confirmation email, call 343-202-1765, or email info@crestviewdental.ca. You may also contact the Information and Privacy Commissioner of Ontario if you have a privacy concern we cannot resolve.
